No matter what goals clients are trying to achieve, writing down their day-to-day activities can help them identify problem areas. People often don't realize what they're doing when it's a habit. Writing it down raises awareness and is the first step toward a solution.
We're going to use personal behavior modification logs with our clients. These logs track exercise and eating habits to help clients identify their behaviors. I haven't seen any forms that I really like, so I'd like us to develop a new form together. I've come up with a list of categories, and I'd like you to mark
